AYURVEDIC MANAGEMENT OF INFERTILITY: EVIDENCE-BASED UPDATES FOR INTEGRATIVE REPRODUCTIVE MEDICINE

Abstract
Infertility affects a considerable portion of the global population (10–15%) and frequently presents as a complex, multi-factorial challenge complicated by metabolic comorbidities such as Polycystic Ovarian Syndrome (PCOS) and significant psychosocial stress. While Assisted Reproductive Technology (ART) offers substantial solutions, its success rates are often limited by intrinsic factors like poor gamete quality, diminished ovarian reserve (DOR), and inadequate endometrial receptivity. Integrative Reproductive Medicine (IRM) provides a paradigm for combining biomedical approaches with holistic traditional systems, such as Ayurveda, based on rigorous evidence.
Objective: This review critically synthesizes the classical Ayurvedic concepts of Vandhyatwa (infertility) and its comprehensive management principles (Shodhana, Shamana, Rasayana) with contemporary clinical data derived from indexed biomedical literature. The primary aim is to establish an evidence base for Ayurveda’s role as an efficacious primary or adjunct therapy within modern IRM protocols.
Data Sources: A comprehensive review was conducted, encompassing randomized controlled trials (RCTs), systematic reviews, detailed case reports, and pharmacological studies retrieved from electronic databases including PubMed, Scopus, and the Cochrane Library. This analysis was anchored by fundamental concepts delineated in classical Ayurvedic texts, specifically the Brihat Trayi.
Synthesis/Findings: Ayurvedic interventions demonstrate significant efficacy across major infertility factors. Targeted purification therapies, such as Uttara Basti (intrauterine medicated douching), have been clinically shown to restore tubal patency in up to 80% of treated patients.Furthermore, Rasayana phytotherapies, including Withania somnifera (Ashwagandha) and Asparagus racemosus (Shatavari), improve key reproductive parameters. W. somnifera enhances sperm concentration and motility, while A. racemosus supports ovarian function, with reported improvements in Anti-Müllerian Hormone (AMH) levels, a crucial marker for ovarian reserve. These biological effects are largely attributed to their adaptogenic properties, reduction of oxidative stress, and modulation of the hypothalamic-pituitary-gonadal (HPG) axis.
Conclusion: Ayurveda offers scientifically plausible, holistic, and efficacious interventions that address the systemic root causes of infertility. The integration of evidence-based Ayurvedic treatment protocols, including specialized local therapies and systemic Rasayanas, holds promising potential to enhance patient outcomes, particularly for conditions refractory to conventional monotherapy, and to simultaneously mitigate the recognized psychological burdens associated with infertility treatment.
